Have there been any documented cases of professional musicians losing their temper during live performances and making an audible outburst on stage?

When performing a solo tour in 1984, Roger Waters enlisted Eric Clapton to be his guitar player.

During the shows, everyone would be seated until the guitar solo. Then the audience would stand and listen to Clapton until the solo was over, at which time they would applaud even while Roger had begun singing again.

Roger got tired of this. They were supposed to be there to hear his music and not just to watch Clapton play guitar.

After one solo, a seething Waters caustically said to the audience, “The GREAT Eric Clapton” and immediately regretted it. Clapton was mortified.

Backstage, Waters apologized profusely to Clapton, who said it was fine.

For his next album, Waters politely invited Clapton to play on it. Clapton politely stated that he was not available.

I forgot about this one. One of the roadies decided to spread out his Burger King on top of Pete’s amp. Pete was not the least bit happy about it.
