My choice would be strongman.

After becoming the first man in history to deadlift 500kg, Eddie Hall bled out of his nose and ears, had hugely high blood pressure, temporarily lost his vision and had memory problems for weeks.

In the photo above, Eddie Hall weighed roughly 440lbs at 6′2″, giving him a BMI of 56. The strain on your heart at that weight is huge, no matter how much of it is muscle. He had to eat more than 12,000 calories daily to maintain it.

Strongmen lift huge weights, weigh way more than is healthy and take plenty of performance-enhancing drugs. Steroids, growth hormone and insulin, to name a few.

I haven’t even mentioned the broken fingers, torn muscles and ligament damage that are part of the job if you’re a professional strongman.

The final straw for Eddie came when his doctor told him, “If I lined up all 70 million people in the UK, I’d pick you as the most likely person to have a heart attack or stroke right now.” He quit the sport soon after winning World’s Strongest Man in 2017. Today, he weighs a very slender 350 lbs.

Those currently competing in strongman are still massive, still take drugs and still have some of the highest risks of cardiovascular disease in the world.
